2. Complete W-9 form

Please return to Sherre Ross by mail or e-mail at sross@sbtinsurance.com along with a copy of your license and proof of E&O coverage. No minimum production level is required. The Managing Agency Group programs require a separate agreement, and information will be provided by our CT and NY offices.
